Food allergies may be merely annoying, or they may be life-threatening. About 30,000 Americans rush to emergency rooms every year to receive treatment for serious allergic reactions to foods. For about 150-200 people, these reactions are fatal. People with food allergies must be careful about what they eat. They also worry about “hidden” allergens that might sneak into their food.
What exactly is a food allergy? It’s a specific type of adverse food reaction that involves the immune system. The body produces an allergic antibody to a food. Then, the next time that food is eaten, the antibody binds with the food and causes an allergic response.
Signs and symptoms may develop within minutes or within an hour after eating the offending food. Here are some of the most common:
• itching, skin rash, hives
• swelling of the lips, face, tongue, throat or other body part
• nasal congestion, wheezing, or difficulty breathing
• abdominal pain with diarrhea, nausea, and/or vomiting
• lightheadedness or dizziness

The FDA requires food manufacturers to list the presence of the most common food allergens on their labels. If the product contains one of these foods or a protein from one, it must be listed.
Any domestic or imported food regulated by the FDA must supply information such as “contains eggs” on the label. If a food may accidentally come in contact with one of these allergens during the growing, harvesting, or manufacturing process (for example, if peanuts are processed in the same plant), the manufacturer is not required to mention this on the label, but many do list the possibility of trace amounts of a common allergen. Errors of omission do occur, but, when government inspectors notice that a product contains an unlisted allergen, there is a recall of the product.
The government’s labeling requirements do not protect everyone with a food allergy. The 8 most common offenders listed above account for 90% of all food allergies. However, more than 160 different foods are known to cause allergic reactions.

• Adults usually keep their food allergies for their entire lives, but children sometimes outgrow theirs.
• The highly allergic person can become sick from eating even a very tiny amount of a food allergen, for example, a small piece of a peanut kernel.
• Some allergic food reactions occur only after exercise.
• A positive test for IgE antibodies to a particular food does not necessarily mean that the person will experience an allergic symptom after eating that food. In fact, there’s less than a 50% chance that the person actually has that food allergy.
Food allergies are different from food intolerance. Some food ingredients that can make a person sick because of intolerance are histamine, lactose, food additives (such as MSG and sulfites), and gluten.
A health care provider should be consulted for help in diagnosing and treating food allergies. The websites of the National Institute of Allergy and Infectious Diseases have a wealth of information on these topics on diagnosis and treatment of food allergies, but, ultimately, the allergic person’s best source of information is his/her own doctor, who can take a medical history, do an exam, do allergy tests, and consider the patient’s other medical problems before treating the allergy.
If you are a person with a history of dangerous allergic reactions, two possibly life-saving recommendations are the following: 1) wear a medical alert bracelet or necklace that states the food allergy you have and possible severe reaction(s) to it; and 2) consult your doctor about the advisability of carrying an auto-injector device containing epinephrine (adrenaline) that you could administer to yourself if necessary. (You will need a doctor’s prescription for this.)

Anallergy isan abnormal reaction of the human immune system, when it wrongly identifies certain allergens such as pollens, foods, drugs, dust, etc to be harmful foreign substances and produces antibodies against them. When these antibodies are produced in excess, they release histamine and other chemicals in your body, which in turn results in some allergic reaction. Allergy manifests itself most frequently in the respiratory tract or the skin – these manifestations of allergic conditions include allergic rhinitis, eczema, hives and hay fever. Severe reactions include violent cough, cyanosis, fever, pulse variations, convulsions and collapse. Allergic reactions may be acute, chronic, immediate or delayed and the agent causing the allergy is to be identified to provide apt treatment – avoidance, medication or immunotherapy treatment.

Percutaneous and intracutaneous allergy tests are accepted as medically necessary and are covered by most insurance providers. However, there are certain limitations imposed:
The number of scratch, puncture or prick allergy tests eligible for reimbursement per year is 70 (CPT code 95004)
The number of intracutaneous allergy tests eligible for reimbursement per year is 40 (CPT codes 95024 and 95028)
SET testing is considered medically necessary and is covered up to 80 tests per year (CPT codes 95010, 95015 and 95027).
Patch test, photo patch test, mucous membrane test, bronchial inhalation challenge test, and food ingestion challenge test are also considered medically necessary and covered for the diagnosis, evaluation and treatment of allergies. Other allergy testing procedures including sublingual testing, provocative testing, and Rebuck skin window test are not covered because they are considered experimental or investigational. These are to be reported with the CPT code number 95199.
Tests such as Leukocyte histamine release, Prausnitz-Kustner test, Cytotoxic food testing (leukocytotoxic test, Bryans test) and Conjunctival challenge testing (ophthalmic mucous membrane test) are usually excluded from reimbursement.
In vitro testing involves blood tests to identify the presence of specific IgE antibodies to a particular antigen. Procedure codes for allergy tests are usually reimbursed per test for the total number of tests performed.
Allergy laboratory testing includes CPT codes 86000-86999. 86003 and 86005 are codes used to signify allergen specific IgE determinations. RAST, MAST, FAST, ELISA, and ImmunoCAPtests are indicated when percutaneous testing of IgE-mediated allergies cannot be done. Radioallergosorbent testing for allergies has to be reported with code 86003 (allergen specific IgE; quantitative or semiquantitative, each allergen).Service providers reporting with this code have to give supporting documents to validate the medical necessity for the allergy testing procedure, explaining why other routine allergy tests were unsatisfactory. These tests are reimbursed in the following situations:
When skin tests cannot be performed routinely due to conditions such as infancy, extensive eczema, icthyasis or dermographia
When the skin test has proven inconclusive
When the patient is under medication that might interfere with skin testing, but can’t be asked to discontinue
When the patient has systemic reaction to skin testing
If medical necessity is not validated, code 86003 might not be reimbursed. Apart from CPT codes, the claims should also contain all relevant ICD-9 and HCPCS codes for full reimbursement.
Allergy immunotherapy includes avoidance therapy, pharmacologic therapy and immunotherapy. Avoiding the allergen responsible for causing allergy is the most effective treatment. Allergy immunotherapy also known by the terms hyposensitization, desensitization, allergy injection therapy, or “allergy shots” is recommended for patients

Allergy immunotherapy desensitizes the patient to the allergen that caused the allergy. It protects the patient against the allergic symptoms and inflammatory reactions. Allergy shots are usually provided in a medical setting and contain increasing doses of the allergen. The healthcare provider must accurately document the allergy shots given to the patient to be properly reimbursed. To avoid claim denials, it is important that the patient is examined by a physician; in addition, the examining physician has to prepare the antigens, develop a treatment plan and dosage schedule.
In allergy immunotherapy, the CPT codes covered include 95115-95170, 95199. If other identifiable services are given during office visit, office visit codes can be used in addition to allergen immunotherapy codes.
When percutaneous or intracutaneous sequential and incremental injections (95010, 95015 or 95027) and single injection (95004 or 95024) tests are provided on the same date, all these codes may be reported if the tests are different allergens or different dilutions of the same allergen. The number of separate injections are to be reported, do not report both a single injection test and a sequential and incremental injection test for the same dilution of an allergen. SET testing can be reported and will be reimbursed on a per allergen basis.
The patient’s medical record must clearly document the medical necessity for the treatment provided. Medicare Benefit Policy Manual provides details of services covered, codes and other relevant details helpful in accurate coding. A standardized CPT code is assigned for every medical procedure and task. Medical coders have to be thorough with the reimbursement policies of different insurance providers
Symptoms and Home Remedies for Food Allergy Treatment
Either food allergy or food intolerance affects nearly everyone at some point. When people have an unpleasant reaction to something they ate, they often think that they have an allergy to the food. Actually, however, only up to 3% of adults and 6%-8% of children have clinically proven true allergic reactions to food.
This difference between the prevalence of clinically proven food allergy and the public’s perception of the problem is due primarily to misinterpreting food intolerance or other adverse food reactions to food as food allergy. A true food allergy is an abnormal response to food that is triggered by a specific reaction in the immune system and expressed by certain, often characteristic, symptoms.
Other kinds of reactions to foods that are not food allergies include food intolerances (such as lactose or milk intolerance), food poisoning, and toxic reactions. Food intolerance also is an abnormal response to food, and its symptoms can resemble those of food allergy. Food intolerance, however, is far more prevalent, occurs in a variety of diseases, and is triggered by several different mechanisms that are distinct from the immunological reaction responsible for food allergy. People who have food allergies must identify and prevent them because, although usually mild and not severe, these reactions can cause devastating illness and, in rare instances, can be fatal.
The complex process of digestion affects the timing, location, and particular symptoms of an allergic reaction to food. All of the symptoms of food allergy occur within a few minutes to an hour of eating. A food allergy can initially be experienced as an itching in the mouth and difficulty swallowing and breathing. Then, during digestion of the food in the stomach and intestines, symptoms such as nausea, vomiting, diarrhea, and abdominal pain can start. Incidentally, the gastrointestinal symptoms of food allergy are those that are most often confused with the symptoms of different types of food intolerance.
As mentioned previously, the allergens are absorbed and enter the bloodstream. When they reach the skin, allergens can induce hives or eczema, and when they reach the airways, they can cause asthma. As the allergens travel through the blood vessels, they can cause lightheadedness, weakness, and anaphylaxis, which is a sudden drop in blood pressure. Anaphylactic reactions are severe even when they start off with mild symptoms, such as a tingling in the mouth and throat or discomfort in the abdomen. They can be fatal if not treated quickly.

Most allergies to foods begin in the first or second year of life. While some of these reactions may resolve over time (such as allergies to cow’s milk or eggs), other food allergies acquired in infancy (such as allergies to nuts or shellfish) typically persist throughout life. Allergies to milk or soy formula (a milk substitute made from soybeans) sometimes occur in infants and young children. These early allergies sometimes do not involve the usual hives or asthma but rather can cause symptoms resembling infantile colic, and perhaps blood in the stool, or poor growth.
Breastfeeding: Exclusive breastfeeding, that is, excluding all other foods, for at least the first four months of life appears to help protect high-risk children against milk allergy and eczema in the first two years of life. Breast milk contains less protein that is foreign to the infant and, therefore, is less allergenic than cow’s milk or soy formula.
Exclusive breastfeeding should be a consideration, therefore, especially in infants who are predisposed to food allergy. Some children are so sensitive to a certain food, however, that if the mother eats that food, sufficient quantities enter the breast milk to cause a reaction to the food in the child. In this situation, the mothers themselves must avoid eating those foods to which the baby is allergic. No conclusive evidence has been obtained that suggests that breastfeeding prevents the development of allergies later in life.
Special considerations in children: An allergic child who itches, sneezes, and wheezes a lot can feel miserable and, therefore, sometimes misbehave or appear hyperactive. At the other extreme, children who are on antiallergy medicines that can cause drowsiness may become sleepy in school or at home. Parents and caregivers must understand these different behaviors, protect the children from the foods that induce their allergies and know how to manage an allergic reaction, including how to administer epinephrine. Also, schools need to have plans in place to address emergencies, including anaphylactic shock.
Certain nutrients have been found beneficial in the prevention and treatment of allergies. Often the intake of vitamin B5 or pantothenic acid brings great relief to the sufferer. This vitamin may be taken in a dose of 100 mg daily for a month. In such cases, liberal amounts of pantothenic acid helps even though the recovery takes several weeks. A dose of 400 mg of vitamin E taken daily for four to six weeks is also beneficial as this vitamin possesses effective anti-allergic properties.
It is found that five drops of castor oil in half a cup of any fruit or vegetable juice, or plain water, and taken on an empty stomach in the morning, is beneficial for allergies of the intestinal tract, skin, and nasal passages.
Lime is considered an effective remedy for any kind of allergy. Half a lime may be squeezed in a glass of lukewarm water and sweetened with a teaspoon of honey. It can be taken once daily first thing in the morning for several months. This remedy not only flushes the system of toxins but also acts as an antitoxic and antiallergic agent. However, those who are allergic to citrus fruits should not take recourse to this remedy.
One or two bananas a day are useful for those who are allergic to certain foods and who consequently suffer from skin rashes, digestive disorders, or asthma, The fruit does, however, cause allergic reactions in certain sensitive persons and they should avoid it.
A quantity of 500 ml carrot juice or a combination of carrot juice with beet and cucumber juices, has been found beneficial in the treatment of allergies.
